• Church Creek Senior Living is a full-service senior living community offering independent and assisted living residences, Five Star’s award-winning Bridge to Rediscovery Alzheimer’s care program, skilled nursing care, and respite/short stays. With a wide range of exceptional services and a 24-hour staff, we are committed to serving residents’ needs around the clock. Residents enjoy three delicious meals a day, served restaurant-style, in our assisted living, skilled nursing, and Bridge to Rediscovery neighborhoods; extended hours for breakfast and dinner are included in our independent living neighborhood.
