  • What is Affordable Homecare Services? Affordable Homecare Services refers to one-on-one, around-the-clock, personalized care within the home. Affordable Homecare Services are an effective solution for seniors who require 24-hour care and are able to provide a caregiver with food and a place to sleep. Affordable Homecare Services is also a good alternative if you will be on vacation or out of town for a short period of time and would like for someone to be at home with your loved one in your absence. Use the list below to help you understand Affordable Homecare Services and how it affects your family. Designate a space in your home for the homecare provider, this includes the following: •Sleeping arrangements (a separate bed)* •A place to put his or her belongings (dresser, closet, etc.) •Groceries/food (provided by patient and/or family, but prepared by live-in) Rates and Caregiver Responsibilities: Affordable Homecare Services rates vary by location and are subject to change depending on the type of care needed and other factors including number of clients, transportation needs, etc. •The homecare provider is only responsible for the care of his/her patient, not other individuals living in the home. •Homecare can provide the following services**: ◦Bedside monitoring for patients in a facility or in the home ◦Assistance with personal care activities (bathing, grooming, etc.) ◦Reminding (medication, dates, routines, etc.) ◦Assisting with mobility ◦Preparing meals and feeding ◦Providing companionship ◦Performing light housekeeping ◦Running errands (prescription pick-up, dry cleaning, etc.) ◦Organizing, shipping, and reading mail ◦Grocery shopping ◦Escorting to appointments ◦Entertaining (games, crafts, reading, etc.) •What to expect from Sunbrite? Transitioning from the Hospital, Rehab Center, or Facility to the Home: •At Sunbrite Companion Homecare Services, LLC we feel that choosing the right homecare provider is one of the most important steps in transitioning to outside help. To begin this process, a Sunbrite representative will schedule interviews with potential homecare providers to determine if he or she is a good fit for your needs. •After you choose the right homecare provider, a Sunbrite representative will arrange any necessary sessions, including a therapy session at the facility before you or your loved one goes home. During this therapy session, the homecare provider learns necessary information and exercises to continue to develop the techniques used in your or your loved one’s treatment. Once at Home: •Sunbrite coordinates and provides transportation to and from your home in an effort to make you and your family more comfortable. •Once at home we will send a supervisor to do an in-home assessment to complete the required initial paperwork. This initial assessment will include an agreement for services, emergency management plan, and any other case-specific information. •Additionally, a Sunbrite representative will follow up with you and your family 24 to 48 hours after your case begins to ensure satisfaction. •Sunbrite also performs supervisor home visits to evaluate your case and make any needed adjustments. •In the case that your permanent homecare provider needs time off, Sunbrite will work directly with your family to plan for alternative care by finding a substitute homecare provider. You may also choose to care for your loved one on your own until your permanent homecare provider returns. •Sunbrite representatives are available 24-hours a day, 7 days a week to answer your questions. *As required by law, caregivers must have a set period of uninterrupted sleep while staying with a patient.
